Nomination Instructions for FCA Awards

 

FCA Awards of Excellence


These will be presented at the FCA Conference in the Fall.  Entries must be electronically submitted by September 15, 2017.  The following categories will be recognized:

FCA Advocate Award

The purpose of this award is to recognize an individual or organization which has made a significant contribution to the improvement of the counseling profession in Florida.

Supporting documentation required:

  • Exemplary support of or outstanding promotion of the counseling and human services profession.
  • Explain why you feel the nominee should be the person selected to receive this award.

FCA Member of the Year

The purpose of this award is to recognize a member who has made a significant contribution to FCA.

Supporting documentation required:

  • The nominee must be a FCA member a minimum of 3 years.
  • The nominee must hold a master’s degree in the counseling field or have worked within the human services field for a minimum of 5 years.
  • The nominee must have significantly contributed to the growth and development of FCA.
  • Describe the contributions which the individual has made.
  • Supplemental evidence to support the nomination is acceptable.  This information could be contributions to an FCA chapter or division or any national division or office. 

The “Pete Fisher” Humanitarian Award

The purpose of this award is to recognize an individual or organization which has made a significant contribution for the betterment of humanity and the human condition.

Supporting documentation required:

  • Description of the actions, incidents, or behaviors that illustrate the nominee’s caring in a personal way.
  • Description of the actions, incidents, or behaviors the nominee’s caring through social action.
  • Explain why you feel the nominee should be the person selected to receive this award.

The “Jeff Siskind” Leadership Award

As the FCA Executive Director, Jeff Siskind demonstrated exceptional leadership qualities.  This award is designed to recognize an individual or organization who/that has made a significant contribution in leading their Chapter, Division, and Association.

Supporting documentation required:

  • Description of the actions and behaviors that illustrate the nominee’s leadership skills.
  • Explain why you feel the nominee should be the person selected to receive this award.

 

 


 


FCA CHAPTER AND DIVISION AWARDS


All the materials must be submitted from the current calendar year July 1 through June 30.  Chapters may enter more than one award category; one form for each nomination is required.  Application deadline is September 15, 2017.


Outstanding Chapter of the Year

This award is designed to recognize the chapter who best demonstrated the objectives of a FCA chapter.  The Chapter of the Year shall be determined in the Chapter Annual Report along with the following:

  1. Completed Nomination Form
  2. In 1,000 words or less, describe the goals and activities your chapter accomplished by answering the following:

·          What were the goals and objectives of your chapter?

·          How did you determine these goals and objectives (i.e., needs assessment)?

·          What activities and/or programs did you institute in order to carry out these goals and objectives?

·          What was the timeline that was followed?

·          How did the chapter evaluate the success of these activities and/or programs in carrying out these goals and objectives? 


Best Chapter Membership Service and/or Best Membership Recruitment Campaign

The purpose of this award is to recognize the chapter that best promotes the counseling profession or a chapter that creatively recruited new membership to the chapter.  The Best Chapter Membership Service and/or Best Membership Recruitment Campaign shall be determined in the Chapter Annual Report  along with the following:

  1. Completed Nomination Form
  2. In 1,000 words or less, describe the program or service to be considered by answering the following (include supporting documentation and samples where appropriate):

·          A brief description of the program or service and the results you achieved

·          Who was the target audience: (i.e., current members, new members, the community)

·          How did the chapter assess the needs of members and how was this used to develop the appropriate service or program?

·          What were the goals and objectives of the service or program?

·          Describe the process used to carry out the program or project.

·          What was the timeline followed?

·          How did the chapter measure the success of the service or program? (i.e., increased membership, increased attendance, increased revenue, increased visibility in the community)


Best Chapter Newsletter

The purpose of this award is to recognize the chapter that publishes a newsletter in a timely fashion and informs, educates, and motivates to participate actively in chapter activities.  The Best Newsletter shall be determined in the Chapter Annual Report along with the following:

  1. Completed Nomination Form
  2.  In 1,000 words or less, use the following categories to describe the publications to be
    considered.  
  3.  SCAN and email representative copies of the publication. 

·           What is the budget for producing the publication?

·           What is the circulation of this publication?

·           What is the format for this publication? (i.e., 8 ½ X 11”, 12 pages)

·           What is the frequency of this publication? (monthly, quarterly)

·           What are the goals and objectives of this publication?

·           What method of production and distribution is used for this publication?

·           How does this publication serve the chapter’s needs as determined by its goals? 

 

Outstanding Division of the Year:  

This award is designed to recognize the division that best demonstrated the objectives of a FCA division.  The Division of the Year shall be determined in the Division Annual Report along with the following:

  1. Completed Nomination Form
  2. In 1,000 words or less, describe the goals and activities your division accomplished by answering the following:

·          What were the goals and objectives of your division?

·          How did you determine these goals and objectives (i.e., needs assessment)?

·          What activities and/or programs did you institute in order to carry out these goals and objectives?

·          What was the timeline that was followed?

·          How did the division evaluate the success of these activities and/or programs in carrying out these goals and objectives? 


Best Division Membership Service and/or Best Membership Recruitment Campaign

The purpose of this award is to recognize the division that best promotes the counseling profession or a division that creatively recruited new membership to the division.  The Best Division Membership Service and/or Best Membership Recruitment Campaign shall be determined in the Division Annual Report along with the following:
